The Policy

The plan finder says only Cigna and Blue Care / Blue Advantage cover both Aimee and Nicole. The cheapest of those is $430.87/mo. It looks expensive, but it's actually the cheapest year you can have, because the $350–365 plans make you pay Aimee and Nicole in cash.

Cigna Connect Bronze CMS Standard
$430.87/mo · $50 per Aimee or Nicole visit from day 1 · HSA-eligible. Plan ID 73943NC0070046.
~$7.2k/yr
All-in at your current pace: 24 Aimee + 12 Nicole visits plus meds.
~$9.5k/yr
All-in for the "cheap" $365 Ambetter plan once Aimee and Nicole are paid in cash. That's about $2.3k a year more.
→ ~$300/mo
What the Cigna premium can drop to if you shelter ~$14k of income in a Solo 401(k) + HSA (§02).

01The answer

The premium is the wrong number to compare. Aimee and Nicole together are ~36 visits a year, so what you pay per visit outweighs a $66/mo difference in premium.

Calculated with your defaults below: 24 Aimee sessions, 12 Nicole visits, 1 PCP visit, oxcarbazepine $15/fill cash. "Worst" assumes a terrible year: premium + out-of-pocket max + any visits paid in cash.

My pick

Cigna Connect Bronze CMS Standard

$430.87/mo
Aimee, Nicole
$50 per visit from day 1 (outpatient mental health). Both show Covered.
PCP
$50, and your yearly physical is free. Jenna isn't covered, so pick a Cigna PCP. You barely see her anyway.
Generic Rx
$25. Oxcarbazepine is probably cheaper in cash (Cost Plus / GoodRx), which is fine. Cash purchases just don't count toward the deductible.
Deductible / max
$7,500 / $10,000. "Easy pricing" means it's a federal standardized design.
Bonus
HSA-eligible: pay your copays with pre-tax money, and it lowers the income your subsidy is based on (§02).

Why not the others

  • Ambetter / Oscar / Blue Home ($351–365): Aimee and Nicole aren't covered, so you'd pay $150 + $119 per visit in cash (~$5k a year). The $66/mo you save on premium only covers that if you see Aimee fewer than ~8 times a year.
  • Cigna Bronze 5500 ($430.49): 38¢ cheaper, but the mental-health copay is $115, which adds ~$2.3k a year. Avoid.
  • Cigna Silver CMS Standard ($536.94): $40 visits and a lower out-of-pocket max ($8,900). That costs ~$1.3k a year more than the bronze to save $10 a visit. It's only worth it if you get a subsidy that makes silver cheaper.
  • Blue Care Bronze Standard ($536.43): the same $50 design as the Cigna pick, and it also covers Jenna. It's $1.3k a year more just to keep a PCP you see once a year.
  • Blue Care / Blue Advantage Bronze Basic ($514–530): a $95 copay. No.

02Making $430 hurt less

You can't get a lower premium on any plan that covers Aimee and Nicole. What you can change is the subsidy and your taxes, and both depend on your MAGI. At $57.6k you're just above where help starts (~$54k).

Assumes the benchmark (second-cheapest silver) is ~$460. Look at the 2nd price when you sort silver by premium and tell me the number. Subsidy uses the 2027 IRS table. Because your premiums are also deductible, real MAGI ends up a bit lower and the subsidy a bit higher than shown.

Why sheltering works

  • You don't spend the money; it goes into your own HSA or 401(k).
  • Every dollar below ~$54k MAGI earns ~10–20¢ of subsidy, plus ~16¢ of income tax saved (12% federal + ~4% NC).
  • HSA money pays your $50 copays pre-tax, and anything left over carries over forever.
  • Set it up at Fidelity: a free HSA, plus a free Solo 401(k) or SEP-IRA. You can make 401(k) contributions up to your tax deadline.

Tax back regardless

  • Self-employed health insurance deduction: as a 1099 worker you deduct your premiums. ~$5.2k × ~16% ≈ $830 back at tax time.
  • So the Cigna pick really costs about $6.3k after tax in a normal year, before any sheltering.
  • The catch with advance subsidies: if your income ends up higher than you estimated, you repay all of it. Estimate MAGI honestly, or take the subsidy as a refund at tax time instead.

03Before you enroll

  1. Confirm with the humansPlan-finder directories are often wrong. Email VOB@animosanopsychiatry.com and ask Aimee's office: "Are you in network for Cigna Connect (individual marketplace HMO) in 2027?" Both list Cigna on their own sites, which is a good sign.
  2. Add oxcarbazepine in the plan finderCheck it's on Cigna's drug list (formulary). If it's a cheap generic, cash at Cost Plus may beat the $25 copay anyway.
  3. Ask your mom when Walgreens drops youDec 5 or Dec 31. If it's Dec 5, COBRA can cover the gap retroactively, and you only pay if something happens.
  4. Enroll by Dec 15 for a Jan 1 startDo it in an application, not window shopping: the application gives the exact price. Put your honest 2027 income estimate. Pay the first premium immediately.
  5. Pick a Cigna PCP near ConcordGet your free physical in early 2027.
  6. Smoke or vape?Still unanswered. Tobacco use can raise the premium a lot.

Care

Income & sheltering

Typical = premium after subsidy + copays + cash visits + meds. Worst = premium + out-of-pocket max + cash visits. Unchecked = out of network, paid at the cash rate. Copays count toward the out-of-pocket max. Catastrophic is a guess (none showed in your paste) and can't use a subsidy. Tax savings aren't included.

05Background

Rules that shaped this

  • The COVID-era extra subsidies expired; 2027 uses the original ACA table. You're expected to pay 10.22% of MAGI at 300–400% FPL, and get no help at all above $63,840.
  • Since 2026, every bronze plan is HSA-eligible.
  • If you take subsidy in advance and your income comes in higher, you repay all of it. There's no cap anymore.
  • Enroll Nov 1 – Dec 15 for a Jan 1 start. Turning 26 on Dec 5 also gives you a special enrollment window as a backup.

Skip / extras

  • Don't buy health-sharing ministries or short-term plans. They exclude mental health and conditions you already have.
  • Dental isn't in health plans. Options: a standalone dental plan on HealthCare.gov ($20–40/mo; 2 cleanings, ~$1–1.5k annual max, waiting periods for bigger work), or a dentist's in-house membership (~$300/yr). For cleanings only, the membership usually wins.
  • Cigna turned out to still be selling NC marketplace plans for 2027, contrary to an earlier report I found. The plan finder is the source of truth.
